YOUR FIRST INTEGRATIVE AESTHETICS VISIT

• If possible, please complete the consent and intake forms online prior to arrival.
• Please bring a picture of yourself in your younger years. This will be used in your consultation to show Dr. Adams your concept of ideal results.
• You and Dr. Adams will collaborate on a treatment plan that works for you.
• Here are the forms that will need to be filled out prior to treatment by Elizabeth Adams MD. Either you print them off and bring them into the office the day of your appointment or we will provide you a copy prior to your appointment.
